THE LIVING WILL MUST BE SIGNED AND WITNESSED in the presence of two witnesses. These adult witnesses must be over the age 18 and present at the same time. They DO NOT NEED to a special authorised person.

To successfully execute your Living Will you must follow these instructions in order for your Living Will to be a legal and valid document.

1. Complete the date.
2. Sign the Living Will with usual signature above where your name is noted. – YOU MUST sign your Will in the presence of two adult witnesses over the age of 18
3. The two witnesses must then, in your presence, sign their names, using the same pen, in the space provided adding their full names, address and occupation.
